Buyout Forms

Steps for submitting requests

  1. Download the appropriate PDF form above.
  2. Open in Adobe Acrobat only.
  3. Business managers should fill out the form.
  4. E-mail the form to your department head and instruct them to review the form, and sign in Department Head signature block, and save form. 
  5. After saving, they should send an e-mail to the next person that needs to sign the signature block with similar instructions. 

 

NOTE

If multiple funding sources are used, you will need to contact fcbo@uga.edu to get the appropriate spreadsheet for indicating funding sources.
