Summer School Preplanning Instructions

For summer pre-planning, please fill out your summer school funding request on one or both of the following two forms as appropriate. Please note that the following forms are PDF and must first be saved to your computer in order to retain information within the form. Click here for directions for saving a PDF to your computer.
